Parents/guardians may enroll their children in district schools at the Brisbane School District Office.  The District Office is located at 1 Solano Street, Brisbane (inside Lipman Middle School)  415.467.0550

Open enrollment for the 2019-2020 school year starts February 1st, 2019.  Please come to the District Office between 8:30 AM to 4:00 PM to register.  Enrollment applications will also be taken at Panorama Elementary School between 8:30 AM to 4:00 PM on February 1, 2019. Panorama Elementary School is located at 25 Bellevue Ave, Daly City.

After February 1, 2019 applications should be submitted to the District Office unless you make an appointment to meet with a District Office Staff Member at Panorama Elementary School.

The following documentation is required to register:

To register your child for school, you will need the following on registration day:
1. A completed 2019-2020 registration form (Forms will be available at registration site. Once available, forms will also be posted online.) *
2. Photo ID of parent/guardian *
3. Proof of the child’s age (bring an original birth certificate, passport, or hospital certificate) *
4. Child’s most recent immunization record * (even if not compliant with immunization admission requirements)
5. TWO (2) proofs of residency in the district (see below, each line item counts as one proof) *

Acceptable proofs of residency include:

  • Current PG&E bill and one other recent utility bill (water, garbage, etc.) - Cell phone bills will not be accepted

  • Both current automobile registration and current auto insurance policy displaying your address (count as one proof)

  • Mortgage statement with homeowner’s insurance

  • Rental property contract, lease, or payment receipts within 45 days

  • Section 8 agreements, must be current

  • Correspondence from a government agency (not the DMV) within 45 days

  • Pay stubs within 45 days or State/Federal Income Tax filed within the past 12 months with W2 form(s) attached

  • Other documents may be acceptable on a case-by-case basis and may require notarization.

The following required documents may be submitted after initial registration, but before August 1, 2019:

*Due at time of registration. If you do not have all REQUIRED documents your application will not be accepted and you will need to return when you have all the REQUIRED documents.

**A completed student health exam form required to start school is due by August 1, 2019 or at time of registration, whichever is the later date.  The State of California requires that all students have a physical examination performed by a physician within 18 months prior to starting 1st grade or within 90 days of starting the 1st grade. If you are registering for Kindergarten, a health exam form completed after March 1, 2019 will fulfill the requirements. If you are registering for Kindergarten and present a health exam form dated before March 1, 2019 or if you are registering for Transitional Kindergarten, then you will need to resubmit another health exam form after March 1, 2019 and before the 90 days of starting the first grade.
